Customers have expressed that while their vehicle is in for repairs is a perfect time to have additional items completed such as Clear Mask, Window Tint, PDR Repair & Headlamps restored. Having these items completed at the same time your collision repair is taking place will save you time, money and is very convenient. Ask your consultant if you qualify for a preferred discount on these items should you have them completed while your vehicle is in for repairs.

Authorization to Repair/Rental Car Notice*

I hereby authorize all original & supplemental repairs to my vehicle mentioned above, along with any necessary parts and/or materials required for the repairs. I also authorize the operation of my vehicle by BodyWorks for testing, inspection or delivery. I acknowledge and agree BodyWorks cannot be responsible for loss or damage to my vehicle, from thefts, accidents, hail, fire or any causes beyond their control. I further agree to remove all personal items from the vehicle to protect against the same. A claim through my insurance company will be necessary in the vent of such occurrences listed above. I understand storage fees may be applicable of $50 per day if my vehicle is not picked up after three days of the completion of repairs and for all total loss fees. I understand that my deductible is to be paid to BodyWorks in full prior to my vehicle being released. Proper ID must be presented in person when credit or debit cards are the forms of payment to be used. Personal checks are NOT accepted. In the event your vehicle is total loss; a $375 admin fee is assessed which covers all the administrative and carrying cost associated with your vehicle which is considered reasonable and customary. If your vehicle requires disassembling and you decide not to have repairs performed or completed, there will be a disassembling fee, and administrative fee, and a storage fee applied that will be your responsibility. Insurance companies and/or your policy may not cover these costs. If you want to repair your total loss vehicle you must meet with the Director and Estimator to review all cost associated with taking on such repair. No total loss vehicle will be repaired by BodyWorks if it is a structural total loss. Vehicles disassembled for assessing damage in many cases cannot be re-assembled safely and therefore will not be performed by BodyWorks. BodyWorks does not pre-order parts unless approved by the Director in advance and parts deposit will be required (Total of Parts). If we order parts and/or begin repairs using a BodyWorks estimate, we cannot accept any insurance company estimate or checks for payment. You must advise BodyWorks if you have an estimate or check from the insurance company. Thee will be a restocking fee if job is cancelled after parts have been ordered. Rock Chips are also NOT covered by your insurance company. If you want them repaired, you must have this completed before your vehicle is painted. Please ask your estimator for details and preferred pricing. Ask your estimator for the latest Auto Spa discounts. All insurance Checks received for your repair will be deposited into your customer account immediately upon receipt. There can be many unforeseen delays in the completion of your repairs. Therefore, you may want to consider a lower cost per day rental car to insure you have enough and/or additional rental days available while your vehicle is being repaired. We can only estimate a target completion date. In many cases you can double the amount of rental days simply by choosing a lower cost vehicle per day. Remember, an accident can be an inconvenience and when your rental runs out it does not mean your vehicle should be complete. Please check your rental car coverage. While we realize choosing a lower total coverage allows you to have lower premium, it will not help you when your rental runs out and your repairs are not complete. If you did not choose the maximum rental coverage available from your insurance company and you do not choose the lowest cost per day vehicle available to help add additional rental days, BodyWorks by MMI, Inc. will not participate with any rental charges.

I hereby authorize BodyWorks to endorse on my behalf all insurance checks and/or drafts received for payment of repairs completed on my behalf whereon my signature would be required. I further authorize to pay BodyWorks directly and understand that any drafts, checks and/or monies received will be applied to the outstanding balance on my account. All insurance Checks received for y our repair will be deposited into your customer account immediately upon receipt. In the event there are any reimbursements necessary, all monies have to be received for the complete repairs and all drafts and/or checks will require a TEN-WORKING DAY hold to allow sufficient time for them to process and clear the banking institution. Once this time has expired and all the drafts and/or checks have cleared, a reimbursement check will be requested and in most cases, can be ready for pick up or mailing by the next business day. No Personal checks accepted. Only confirmable bank checks, debit cards, credit cards accepted.

“HAIL DAMAGE NOTICE” Hail damage is classified as an “act of Mother Nature” and is imply out of your control and ours. In the event a hail storm passes through our area and your vehicle is staged outside and would sustain additional damage as a result, you would be required to file an additional claim through your insurance company. “ROCK CHIP NOTICE” Rock chips are not covered by the insurance company. They are considered previous damage by the insurance company. Rock chips must be repaired prior to painting your vehicle. Should you not pay to repair the rock chips you will see them when you pick up your vehicle. Ask your estimator for pricing to have them repaired so you won’t see them. “BRAND NEW OEM PARTS” BodyWorks ONLY uses Original Equipment Parts (OEM) for your repair. However, your policy may or may NOT allow for them whenever Alternative Parts such as aftermarket, junkyard, used, re-conditioned or refurbished parts are available. There will be a parts price difference between brand new OEM parts and alternative parts the insurance company has located that you must be responsible for whenever the insurance company w ill not pay for them. “AUTO RESTYLING CENTER” Customers have expressed that while their vehicle is in for repairs is the perfect time to have additional items completed such as Clear Mask, Window Tint, Door Dings and headlamps polished. Please ask your estimator to explain and provide preferred pricing. “REMEMBER” Remove your garage door remote, remove all personal items, remove all items from your trunk, must have at least ¼ tank of uel and your vehicle should be clean so we can see all the damage required by the claim to be repaired. “NO PERSONAL OR BUSINESS CHECKS ACCEPTED” For your convenience BodyWorks accepts all major credit cards and debit cards. All insurance checks submitted will be endorsed & deposited immediately for the sole purpose of applying them to your account at BodyWorks for the repair. In the event there are reimbursements necessary, all checks submitted require a ten-working day time period to allow sufficient time for them to process and clear the banking institution. Reimbursements can be requested after this time has expired. Please allow time for them to be mailed from our corporate office in Charlotte North Carolina. “DROP OFF – WALK AROUND CHECK IN” When you drop off for repairs please allow sufficient time for you and your BodyWorks Staff member to complete a vehicle check in and walk around with you. Should you not perform the walk around check in because you don’t have time, BodyWorks will record and take pictures of all damage to include any non-related damage as well. BodyWorks will notify you should we find any un-related damage for your approval to move forward with repairs. Your vehicle repairs will not be started until we receive written approval from you. BodyWorks will not be responsible for any damage recorded un-related to your claim.
